What is a Care Manager?
A Care Manager is a professional facilitator who offers advice and provides information about the variety of available options for elder care and other services for aging individuals. Care Managers not only offer their expertise in the services available; they also know the quality and costs of different providers.
Similar to the way that a Financial Advisor helps their clients sort through the various ways to manage their wealth, a Care Manager helps aging adults and their families understand and choose the best solutions for care.
Our Unique Approach
As the common adage goes “two heads are better than one”, our unique, collaborative approach to Care Management ensures our clients have the highest quality of life, health and function possible.
Our team is comprised of a Certified Senior Advisor, a Nurse and a Physical Therapist, with over 60 years of combined experience. We believe in the power of the multi-specialty approach and collaboration when assessing and addressing challenges with our clients, because there are many unique layers to each situation. With our approach, you have the resources of a skilled team of medical professionals who will address your situation from three perspectives: Quality of Life, Health and Function. Our Care Management clients benefit from the support and expertise of not only their primary Care Manager, but from our entire team any time they need us.

The first step we take is to perform an assessment to identify the elder's needs, problems and eligibility for assistance. We then suggest a plan to meet those needs. The plan may include:

Providing education, advocacy, and support for the elder and their family

Assisting the elder and their family in identifying their wishes and goals for care

Assessing medical, financial and legal issues, and offering referrals to specialists in order to prevent future problems and to preserve assets

Arranging and monitoring in-home services to ensure quality & compatibility with the service provider, such as home care

Frequent evaluations of the client's circumstances & reassessments are performed as needed.

Health Care Advocacy

Accompaniment to physician's visits if needed

Collaborate with the client's care team to ensure the best quality of care

Assisting with moving an elder to a care facility if home is not safe

Alerting families if there is a change requiring more assistance, or a problem that needs to be solved

Serving as a liaison between elders and their families, especially if family members are not close by
